Award Nomination Forms
Nominations are now open for the Optometric Physicians of Washington (OPW) 2026 Annual Awards and Student of the Year Award! Use the links below to submit your nominees for the Doctor of Optometry of the Year, Young Doctor of Optometry of the Year, Jack Hale Lifetime Achievement Award, and Robin Cameron Memorial Award. The deadline for Student of the Year nominations is Monday, March 9, and all other award nominations must be submitted by Friday, March 13.
Help us honor outstanding leaders and qualified candidates by using the form links to submit a nomination for the following awards:
- Doctor of Optometry of the Year: This award honors the accomplishments of an OD who has been in practice more than 10 years.
- Young Doctor of Optometry of the Year: This award honors the accomplishments of an OD who has been in practice less than 10 years.
- Jack Hale Lifetime Achievement Award: This achievement award may be given to anyone (optometric physician or not) who has distinguished himself/herself in the spirit of Dr. Hale's long-term service to optometry and the community at large.
- Robin Cameron Memorial Award: This award is to encourage active participation in the association by a new member, who has been an OPW member ten (10) years or less.
There are so many of our colleagues who are excellent candidates for the work they have done in their communities, in optometry, or both! Let's make sure that they are nominated so they may receive the recognition they deserve. These accomplishments not only bring honor to the recipient but also respect to the practice and profession of optometry.
Please submit your nominations by Friday, March 13, 2026. Awards will be presented to the recipient during the OPW Annual Convention at the Member Appreciation Party (MAP) on Saturday, May 2 — save the date!
